Subjective
Ms. Tina Jones is a 28-year-old African Woman. She presented to the clinic with complaints of sore throat, runny nose, and itchy eyes. These symptoms started about a week ago. According to her the symptoms began all the sudden, and since then it persists and constant. She takes throat drops and plenty of water to help relief the pain of her sore throat. She stated she takes over the counter pain med (Acetaminophen) to help with her sore throat. The sore on her throat is a little worst in the morning and the pain is constant. She describes the pain as mild to moderate. She states that her nose has a clear watery drainage, and it "runs all day", and the drainage is thin and drippy. She denies of pain, sneezing and clog nose. She does not take any medication to relieve the symptoms of her nose. She also has an itchy eye. She denies of any recent illness. She reports of no fever, chills, or night sweat. Denies of being diagnosed with seasonal allergy, but states that her sister has "hay fever." Social History: She denies of using tobacco or being exposed with anybody who use tobacco. She denies of being exposed to allergens or irritants: No fever, chills, fatigue, weakness, or night sweat. Head: Denies of current headache, no trauma or recent head injury, no dizziness or lightheadedness. Eyes: Denies of pain in the eye. She has itchy and watery eyes. her eyes get blurry sometimes when she reads. Reported that her vision is getting worse over the years. Ears: Denies of hearing loss or hard of hearing, no drainage in the ears, no earache. Nose/Sinuses. Denies of clog nose, or pain in the nose, denies stuffiness or sneezing, no bleeding, has runny nose with clear drainage Mouth or Throat: No hoarseness, no bleeding gums, has sore throat Respiratory: denies shortness of breathing or difficulty breathing, no coughing, no wheezing, has an asthma. She uses inhaler at 2 to 3 times a week.
Objective
General: Ms. Jones is a pleasant, obese 28-year-old African American woman in no acute distress. She is alert and oriented. maintains eye contact throughout interview and examination.
• Head: Head is normocephalic and atraumatic. Scalp with no masses, normal hair distribution.
• Eyes: Bilateral eyes with equal hair distribution, no lesions, no ptosis, no edema, conjunctiva clear and injected. Extraocular movements intact bilaterally. Pupils equal, round, and reactive to bilaterally. Normal convergence. Left fundoscopic exam reveals s disc margins, no hemorrhages. Right fundoscopic exam reveals retinopathic changes. Left eye vision: 20/20. Right eye vision: 20
• Ears: Ear shape equal bilaterally. External canals without inflammation bilaterally. Tympanic membranes pearly grey and in with positive light reflex bilaterally. Rinne, Weber, and Whisper the normal bilaterally.
• Nose: Septum is midline, nasal mucosa is boggy and pale bilaterally. No pain with palpation of frontal or maxillary sinuses.
• Mouth/Throat: Moist buccal mucosa, no wounds visualized. Adequate dental hygiene. Uvula midline. Tonsils 1+ and without evidence of inflammation. Posterior pharynx is slightly erythema with mild cobble stoning.
•Neck: No cervical, infraclavicular lymphadenopathy. Thyroid is smooth without nodules or goiter. Acanthosis nigricans present. Carotid pulses 2+, no thrills. Jaw with no clicks, full range of motion Bilateral carotid artery auscultation without bruit.
• Respiratory: Chest is symmetrical with respirations. Lung sound clear to auscultation without wheezes, crackles, or cough
