Here’s a review of Task 1 for D156… Remember, the evaluators will not “grade” your CPE’s (they only check to see that they are there and that they are complete!).  The real grading comes in D156 Task 1…So, here’s some tips to get you started on D156 Task 1:

 

Stakeholder Identification 

A.  Summarize the process you used to identify  three stakeholders who will contribute to the success of your project proposal by becoming members of your project team, including the following information:

•  the technique you use to identify the three stakeholders

•  a brief description of  each stakeholder’s experience or expertise

•  the role  each stakeholder will have in supporting the proposed healthcare improvement project (HIP).

 

Note: Do not include the actual names of people.

Many of these questions contain request for information about the PROCESSES you use to determine your HIP topics, stakeholders, and steps!  So, bring the reader into that discussion!  Here, they want you to tell them about the specific stakeholders you chose (remember, their initials and job titles only), how they will help your project succeed, how you identified those three stakeholders as well as the experience or expertise each one brings to your project (which may be why you chose them) and the role that each stakeholder will have in the project.  Some students find it helpful to present this information in chart format!  Be sure to include all the information the evaluator is asking for: the experience and expertise he/she will bring to the project as well as the role they will have in your project. And, recently, evaluators have been sending task 1 back for sutdnets not including how each of the stakeholders assisted with the Affinity Analysis, SWOT, and Impact analysis.  If you are planning to present this in a table, be sure to add three additional columns (one for the affinity analysis, one for the SWOT, and one for the Impact Analysis)…and explain how each stakeholder was able to assist.  OR, you can provide this information in a narrative under the table .

Needs Assessment

B.  Explain the process you used to determine the need for an HIP, including the following:

•  the method to capture ideas from stakeholders

•  how these ideas were organized into themes                                                                          

•  the process for reaching consensus on the need for a HIP

 

This section of the task asks you to discuss how you captured ideas from you stakeholders, organized those into themes from which to choose your topic, and your method for reaching consensus for your project. This is pretty much a discussion of  the “affinity analysis” required for the CPE’s (and that’s why you always want to complete the CPE’s before you complete the tasks…you often NEED that information to write the task.

 

Problem Identification

C.  Present the problem or opportunity that will be the focus of your project and explain the significance of your project to your target population, your organization, and your nursing practice.

 

Note: Use the problem or opportunity determined by your needs assessment.

 

 In this section, you want to explain  why the recommendation you are making (whatever that’s going to be to benefit patients) is a benefit for the organization, the patient, and nursing practice!  What might the patient see because of your project (improved patient outcomes, improved patient satisfaction, decreased LOS?) What will that improvement mean to the organization?  And lastly, how will nursing practice change in your unit as a result of your project?  You’ll want to begin this discussion with “the significance of the project to….is…”  Then, “the significance of the project to….to the organization is…” and “The significance of the project to…  for nursing practice is…”

 

Problem Description

D.  Describe the background information leading up to the problem or opportunity at your organization.

In this section, you want to explain what has possibly caused this problem in your organization and what effect that patient falls have had for the patients and organization! What has been happening in the organization to cause the need for the project? Is it a staffing education issue?  A knowledge deficit?  A process that needs revamping? Why, in your opinion, is this happening?

 

Impact Analysis and Template

 

E.  Assess the feasibility of implementing the HIP by doing the following:

1.  Submit a copy of the “Impact Analysis Template” that you completed during your CPE as Appendix A of your HIP paper.

      You will always send the reader to the appendix (in the Impact Analysis’ case, Appendix A) …you don’t bring the appendix to the reader.  So, in your paper for E1, you’ll direct the reader to the appropriate appendix before you complete the discussion  about the appendix.

2.  Discuss the feasibility of the HIP using the results of your impact analysis and an interpretation of the impact ratio calculation.

        Be sure you divide your benefits score by your risk score to get your impact ratio. And report what your impact ratio is.  Next, you will explain what that impact ratio means in terms of your project feasibility (an impact analysis greater than 1.0 means that the project’s benefits outweigh the risks) …a high impact ratio will support the feasibity of completing your project for the organization.

 

SWOT Analysis and Template 

F.  Assess the organization’s readiness for the HIP by doing the following:

1.  Submit a copy of the “SWOT Analysis Template” that you completed during your CPE as Appendix B of your HIP paper.

     Just like you did with the impact analysis, you will send the reader to the appendix (in the SWOT’s case, Appendix B) …you don’t bring the appendix to the reader.  So, in your paper for F1, you’ll direct the reader to the appropriate appendix before you complete the discussion  about the appendix. Mathy, you’ve already done one SWOT analysis in your earlier classes…so you’ve already done this once! You are required to have TWO internal and TWO external criteria…but each of those need two strengths, weaknesses/opportunities, threats.  AND those criteria, strengths, weaknesses, threats and opportunities have to align with YOUR specific project (be sure to choose your factors from the list of examples they offer that impact your particular topic).  You want to provide external opportunities and threats for your external criteria…and internal strengths and weaknesses for your internal criteria. Please review the reading and video on page 36 of your class course of study:  I’ve attached the reading and here’s a link to the video as well:  .Take a peek at the example below to help you with yours… as you can see, its OBVIOUS which criteria goes with which strengths and weaknesses…and  all criteria align with that the chosen factors…. they match! There’s also a great example of a SWOT analysis (and other great resources) in your course of study – page 36. It might help for you to review the SWOT analysis you did for D030 to refresh your memory about how the SWOT is completed!

2.   Describe the process used to complete the SWOT analysis, including how stakeholders or SMEs helped identify the factors that may impact the implementation of the HIP.

     This is another process question, so it should be all about how you, your preceptor, and your team decided upon the SWOT criteria and other content of your SWOT analysis.  So put your story-telling hat on here and describe HOW you completed the SWOT with the help from your Preceptor and team!

 

3.  Describe  one internal or external organizational regulatory or  one compliance requirement that may impede or facilitate the implementation of your project.

     You will need to discuss some sort of a specific regulation or compliance requirement from accrediting or regulatory bodies.   Is there an accrediting (JACHO) or regulatory body (CMS or even federal/state law) that has to do with the problem you are trying to solve by your project (HAPI’s)?  Of course there is! That’s the type of requirements that the evaluators want to see discussed here. Then, when you can relate your project to a compliance or regulatory issue, do you feel it’s helpful or hurtful towards getting your project done?  Personally, I think linking your project to a compliance or regulatory issue would  facilitate your project…your leadership and staff are more likely to support your project if it could be related to a compliance or regulatory requirement.
