Health intervention design project

Hometown Health Department (HHD) is having a Health Fair at the local mall to promote all of its
programs. You have been asked to design the booth for your Chronic Disease Self-Management
program.
Before the materials are sent to the printer, your supervisor would like to see:
ï‚· your promotional poster,
ï‚· a brochure to be distributed to the public,
ï‚· and a Reference page (in APA format) to establish your evidence base.
You will need to design an eye-catching poster presenting information about the chronic disease and
its impact (national/state/local) and how HHD is addressing the situation. Presentation Poster

You will also need to develop a brochure to be given to interested community members. The brochure
should include any community sponsors/partnerships (planning model), why it will be successful for
them (behavior change theory), and indicate cultural/group considerations have been taken into
account.
