You are a home health nurse making a routine visit to your 78-year-old patient. Mr. Whittaker, who is usually very jovial and talkative during your visits is quiet and more withdrawn than you’ve ever seen him. When you entered the patients home you were meet by a nephew you had never seen before. The nephew, Jerry, tells you that he is there providing care for “the old guy” while his cousin Ginger (Mr. Whitakers daughter) goes to visit her new grandbaby for a few weeks. You notice during your assessment that every time you ask Mr. Whittaker a question he looks over at Jerry and looks away quickly. Jerry repeatedly answers for him and keeps telling you how well they are getting along.
