Thus, the public healthcare system has posed a challenge to maintain the highest service quality and lowest costs. According to the Food and Health Bureau (2021), the proportion of public expenditure in total healthcare expenditure has increased from 40% in 1989/90 to 54% in 2019/20. While 90% of inpatient medical services in Hong Kong are provided by the public sector, public-private imbalance has brought a heavy medical burden in maintaining a sustainable development. Since 1999, healthcare system reform has already been discussed and suggested by the Hong Kong government and Harvard University. In the 2008 healthcare reform consultation, the government once proposed six supplementary financing schemes, including compulsory private medical insurance and personal medical savings accounts. However, the public generally did not accept compulsory schemes at that moment. In order to alleviate the heavy medical burden caused by an aging population and medical inflation, the government had finally proposed to let individuals purchase VHIS as a long-term supplementary financing. To increase the protection level of existing insurance services, and reduce the over-reliance on public hospitals and balance the public-private medical services. VHIS is a policy to regulate the indemnity hospital insurance plans offered to individuals by insurance companies, and the participation by insurance companies and consumers are voluntary. It consists of two types of insurance plans. First, a standard plan provides basic protection.
