An 86-year-old male parishioner is on hospice care at home, and his daughter, who is a nurse, has been trying
to meet all his physical needs around the clock. The pastor, who made a home visit, calls the faith community
nurse to express his concern that the daughter is becoming “burned out.” How can the faith community nurse

After reading the article,Transforming and Improving Health Care through Meaningful Use of Health
Information Technology, discuss how meaningful use of data from electronic health records can be used to
improve population health. Have you seen connections between data collection gathered from electronic health
records and how you care for patients? Finally, reflect on your nursing experiences to an incidence where the
electronic health record improved patient outcomes. How was the electronic health record used to improve
outcomes? What negative impact have you seen using the electronic health record?

Discuss two areas of difficulty you encountered or two new nursing interventions you learned this week at your
clinical site. You may also choose to share one of each. Please remember to respond to two of your peers.
(My clinical is just calling family members who’s loved ones has recently passed away. I do calls for Vitas
bereavement call center. I check up on the family members to see how they are doing after their loved has has
passed and to see if they need additional supports. Vitas is a hospice agency. I do not call the family to tell
them their loved ones have passed. I call them just to check how they are doing a few weeks after their loved
one have died.)

What is your upcoming week’s detailed schedule at your community practice experience placement?
Were there any placement items/issues that occurred this week that you feel your instructor should be aware of
that are private in nature and are more appropriately shared here than in the discussion board with your
classmates?
Give a brief description of an objective you worked on this week. Make sure to cite at least one reference
showing how your objective relates to the public health knowledge you’ve studied during this course or the
public health course. You may choose to reference your e-text, journal articles, or videos you’ve studied during
these courses or you may find an outside reference on your own to further enhance your public health
knowledge and practices.
Remember, your journal entries are an important aspect of clinical learning as they serve to help you reflect
upon and get the most out of your community practice experience. Therefore, your journal entry should include
